Association of Triglyceride-Glucose Index and Vitamin D Deficiency with Congestive Heart Failure: Evidence from NHANES 2009-2018

Abstract
Background: With global population aging, congestive heart failure (CHF) has become a critical public health issue. Metabolic disorders and inflammation contribute to CHF progression. Nevertheless, the relationships between the triglyceride-glucose (TyG) index, vitamin D status and CHF remain poorly understood in general populations.
Methods: A cross-sectional study was conducted using 2009 – 2018 NHANES data with 9,492 adult participants. Serum 25(OH)D and TyG index were measured. Statistical analyses and multivariate logistic regression were applied to explore their associations with CHF after adjusting for confounding factors.
Results: Elevated TyG index was significantly correlated with increased CHF prevalence. Vitamin D distribution differed across TyG quartiles, yet the association between vitamin D and CHF became insignificant after comprehensive adjustment.
Conclusion: Higher TyG index is an important risk factor for CHF. Although vitamin D status varies with TyG levels, it exhibits no significant mediating effect on the relationship between TyG index and CHF in adults.
